The following paperwork is needed to complete your sleep study. To save time, you can print the Sleep History Questionaire and Consent Form, fill them out, and bring them with you the night of your sleep study. It could be helpful to ask your bed partner, if applicable, to help you fill out these forms.
Sleep History Questionnaire and Consent Form
The morning questionnaire will be completed the morning after your sleep study. This form will be provided to you by the sleep study technician after your sleep study. If you are unable to complete this form in the morning, a copy is available below.
